WA State Patrol Clearance Form

Anyone who will be acting as an assistant coach, team manager, or a parent who plans to assist on the fields or sidelines, however frequent or infrequent, must complete and submit a Washington State Patrol Clearance Form. It's a simple procedure, but is required by the State of Washington.

To complete the form:

• Open the form at: https://watch.wsp.wa.gov/help/CABFORM.pdf
  

• List the requesting agency as:
  Lake Washington Lacrosse Association, dba Eastlake Lacrosse
  (You do not need to complete the rest of the box)
• Check the Non-Profit Business/Organization box.
• Provide your full last, first and middle name, any alias, and your date of birth. We do not need race, SSN or Driver's license (please complete this section entirely).
• If you have a common name (e.g., "Bob Smith") or if having completed a form in the past and were informed that there were names identical or similar to yours in the State database, please list your height, weight, eye color and place of birth at the bottom of the form.
  

Once complete, print a copy for yourself and one to present to your team's head coach or mail it to:
